Use the following links to fill out and electronically sign forms related to City Utility Accounts. All forms open in new browser tab(s). Please note: if the form comes up as a blank screen refresh the screen, and it should show.
New Residential Utility Account:
Fill out and e-sign:
- Residential Utility Services Agreement (please also attach a copy of your driver's license)
- Water Authorization Form (Residential)
New Commercial Utility Account:

- Commercial Utility Services Agreement (please also attach a copy of your driver's license)
- Water Authorization Form (Commercial)
Existing Utility Account:
- Bank Drafting Application -- Bank Drafting, or Direct Pay, allows customers have their monthly utility payment pulled from their bank account automatically. You will be required to upload a copy of a voided check or letter on letterhead from your financial institution.
- Termination of Bank Drafting -- Stop Bank Drafting/Direct Pay of monthly utility bill.
- Temporary Suspension of Sanitation Service -- Residents who spend the winter away from Great Falls can apply to pause garbage services for 2 - 6 months while gone.
