Jury Excusal Affidavit

To request an excusal from jury service, please fill out the excusal request form that is attached below.

• When filling out this form, make sure you include the date you have been selected.
• The Court requires supporting documents to be filed with your request, as we are a Court of record. Supporting documents may include:
  • Travel itinerary, medical documents/doctor's note, current address proof if the excusal is related to your residence being outside of the city limits, military order, hotel reservation, etc.
• Employers are required to allow employees off of work to serve.
• You will NOT be excused if current contact information is not provided.
• Untimely excusal requests will be automatically denied
