Submit Written Statement

To submit a written statement you need to have a case report number.  You can obtain a case report number by filing an on-line report or report an incident to a police officer.

To submit your written statement, print the form attached below, GFPD Voluntary Statement,  fill the form out and submit the form either by mail, or by dropping it off in the lobby of the Police Department.  

Evidence can be dropped off at the Great Falls Police Department or mailed to the Police Department.
