Dispatch PHS Required Documents

  1. Signed and notarized background waivers (provided with the Personal History Statement)
  2. High school diploma or MT HiSET certificate
  3. Citizenship or naturalization papers
  4. Certified copy of your birth certificate (photo copy not accepted)
  5. All marriage licenses and divorce decrees
  6. Any name change documents
  7. Peace Officer Standards and Training (POST) certifications
  8. Color photocopy of your driver's license (front & back)
  9. Tax Information Authorization - IRS Form 8821 (sign and date only)
  10. Military discharge papers (must include discharge status DD-214 long form)
  11. Copies of any other certificates, awards, or commendations you would like considered
  12. Transcripts from colleges, universities, or other institutes of higher learning
