Citizen Complaint & Request Form

The Citizen Complaint & Request Form is a tool for residents to report local concerns and code violations. The form is used to process, expedite, and track complaints or requests for City services.  Complaints and requests are processed in the order received unless it is considered an emergency.

How do I report a complaint?

Complete and submit a Citizen Complaint & Request Form. Please note, forms submitted with invalid or incomplete reporting party information will constitute a false report and not be processed.  

Two Ways to Submit the Form:

  1. Fill out and submit the Online Complaint & Request Form.
  2. Fill out the Print/Submit Complaint & Request Form. The form can be completed electronically with Adobe Acrobat Reader.  (Adobe Acrobat Reader is available free of charge from the Adobe web site.)

Can complaint forms be submitted anonymously?

No, reporting party information is required.*

* This form is a public record. If the complaint results in a criminal charge in Municipal Court for a code violation, this complaint form is considered an initial incident report and is considered a public record, but subsequent investigative materials are considered confidential criminal justice information and are only releasable in very limited circumstances pursuant to MCA 44-5-303.

What is the City’s process for handling complaints?

The submission of the complaint form triggers the investigation/ inspection process. This process determines if a violation exists. If a violation exists, the property owner is notified of the violation. City staff will then work with the property owner to achieve voluntary compliance. On rare occasions, when voluntary compliance is not achieved, the City will pursue legal action. Legal action is always the last resort. Over 90% of cases are resolved in the investigation/ inspection phase with no legal action needed.
